Selecting, adding, removing, and editing URLs

Selecting an application, environment, and URL for a plan just got easier.
Functionality for adding and editing applications, environments, and URLs now lives in the settings page. You can easily get there from the side nav or a link in the plan form page.
We added the ability to easily add URLs to application and environment combinations that already have URLs. The URL list is sorted by last updated time.
